When you complete the evaluation form, we collect the details you give us: name, phone, email, state of residence, the healthcare provider involved and your description of what happened. We also record basic technical data, such as IP address and time of submission, to prevent abuse.
How we use it
We use your information only to evaluate your inquiry and contact you about your case. We do not sell or rent your data. We will only text you if you agree to it, and you can opt out by replying STOP.
Third-party tools
This site loads fonts from Google Fonts, maps from Google Maps, and images and videos from Pexels. Those services may log your IP address when they deliver content. The deadline calculator runs entirely in your browser and sends no data.
Confidentiality
Submitting the form does not create an attorney-client relationship. We recommend not including confidential information until we have spoken.
